Next choose the appropriate new patient information form below for your provider and please fill out prior to your appointment. You will also receive an email including a form of your history and interests to complete prior to your appointment.
Print and bring your new patient information form and signed provider disclosure form, and if your intake form (if you did not complete it online) with you to your first appointment! If you have questions or concerns about office procedures, you may ask them before you sign.
Once established with a provider, you can begin using our site to book your own appointments with your selected provider as well as make payments.
**Note: The initial appointment does not guarantee that your psychiatrist will provide ongoing treatment. This extended evaluation visit is to determine a clear diagnosis, to establish a relationship with your provider, and from there plan and/or suggest next steps in your treatment.
